Welcome to the second edition of our Penrose Health newsletter – The Penrose Scroll. Here are some updates that relate to our community… and if you have any suggestions for the next edition, please let us know here. 😊

LMC & KH slam-dunk DNAs to start the year right

In January 2025, we had a total of 1,000 DNAs across our surgeries; a rate of 8%. For context, the National Average DNA rate is 4%. Reasons for DNAs are complex, and at times are a red flag (see our Significant Events Heads Up article below). However, no matter the cause, they are costly for our surgeries in many ways and can be reduced.

Below are the DNA stats for each of our surgeries over the past few months, so let’s see how they performed, and where there might be room for improvement…

It’s a tie! Well done LMC and KH for achieving the joint lowest DNAs in January.

LMC - an impressive drop from 8.8% in November to 7.1% in January - how did you do it?

KH - staying steady in the 7% range, they just pipped Nightingale at the post, who dipped below 7% near the end of 2024. Bad luck Nightingale - surely you could nudge them out next month?

We’d love to hear what measures you’re using to help reduce DNAs, so that we can share these ideas with others and all teams can move DNAs closer to the national average… or lower!

At long last…

Our brand new surgery websites are live! As of the start of 2025, all websites for our Penrose Health locations are live and fully functioning, so feel free to use them to their max capacity. 🚀

Imminent arrival - new e-learning on the Penrose Academy

The scaffolding is coming down and curtain going up shortly on the Penrose Academy training for Care Navigators. For a sneak preview, this reporter can give you an idea of what to expect…

After grilling leads for the best part of 2024, the L&R Team have developed training guides on all things Penrose Health, to complement external training resources (like Blue Stream and EMIS Academy). It is hoped that this e-learning prepares our newcomers to make the most of the one-to-one time with their leads.

Our new hires can expect to find modules on Roles in Primary Care (to help understand the duties of the different teams), Handling Language Barriers, Handling Challenging Behaviour, Mental Health Calls (to help manage potentially challenging situations) and Medical Terminology (to help gain an understanding of the medical language frequently used in our surgeries). There are modules on the new Pharmacy First Scheme and our External Services, too.

Future plans for the L&R Team? More guides in the Penrose Academy for our other disciplines - Paramedics, Nurses, Pharmacy Team, Care Coordinators. Hope the Leads are ready for the inquisition, but… nobody expects the Spanish inquisition!

What’s New in the Penrose Portal?

The L&R Team try to keep an eye on our communication channels, in the hopes of supporting staff when they have questions and making best use of the resources we have available.  It’s very likely that if one person has a question as to how to do something, than others probably have the same question.

Here’s a question that we spotted this week that we thought would be helpful to answer…

Q. What is the process of doing an urgent referral to anticoag?

A. Referral details can be found in the Services Database under Haematology for our surgeries three boroughs!

Significant Event Heads Up

Significant events are best kept to a minimum. However, when they occur, they present a valuable occasion for learning, and honing our systems.

Here, we discuss recent Significant Events and highlight the lessons learned and actions taken to reduce the risk of them happening again.

1️⃣ Vulnerable patient DNAs and safety netting

Event: June 2024 - A schizophrenic patient did not attend (DNA) for their anti-psychotic depot injection.

This DNA was not escalated appropriately and the patient had not attended for any further appointments for the injection. As a result, in January 2025, six months after the injection was due, a member of the public raised a safeguarding concern for the patient.

Learning points

  • If a patient DNAs, staff should check the necessity of the appointment, and if the patient is vulnerable, flag to the appropriate person (team lead or safeguarding lead).

  • If necessary, Reception should try to contact the patient to rearrange the appointment and keep the task open until the appointment is achieved. If they cannot get the patient in, they should notify the Reception Lead.

  • Clinicians should also be aware of a vulnerable patient who requires treatment in a timely manner, set safety netting reminders for treatment, and flag any concerns to the Care Coordinator and/or Safeguarding Lead.

  • Care Coordinators should follow up with the patient and notify PCMHT/SLAM if the patient is not receiving treatment.

2️⃣ Processing clinical letters

Event: A vulnerable patient was prescribed new medication by diabetic team, but the letter from the team requesting termination of previous medication was not processed for a further 8 weeks, meaning that the patient was receiving two types of medication concurrently.

As this patient had a learning disability, confusion about which medication to take could easily have occurred, potentially resulting in patient taking both and becoming hypoglycaemic. To reassure, this did not occur, but was a possibility.

Learning points

Measures are being put in place to reduce the the burden of processing clinical letters. Pharmacists will have time for checking letters, aiming to process up to 10 letters/day. More work is being done to address this, as it is a priority area for improvement.

3️⃣ High dose Vitamin D error

Event: High dose Vitamin D was issued with instructions “Take TWO once daily for 7 weeks”. This was an error that was done when adding the medication as a result of actioning vitamin D deficiency task from pathlinks team.

The error was identified later, when the patient requested a further supply. Patient was contacted and blood test for potential toxicity performed. Results were within normal range.

Learning points

The system error has been addressed and a new Vitamin D insufficiency actioning pathway has been drafted. Also, all prescribers have been made aware of this issue and asked to double check prescriptions.

4️⃣ Misreading of results

Event: A patient was seen by one of our GPs after having taken a high dose of OTC Vitamin D and a blood test arranged to assess potential toxicity. Test result returned a high but not dangerous level, confirming that termination of supplement was necessary.

The result was misread by results team and construed as deficiency. A note was sent to pharmacy team to prescribe more Vitamin D, and was actioned, with patient being texted to collect prescription. Fortunately, the patient had previously discussed Vitamin D levels with GP and flagged it to the GP, who corrected the error before any harm occurred.

Learning points

Great care should be taken in reading results by all team members, particularly at the point of initiating or terminating medication. If there is any doubt or concern, it must be raised with a team lead or GP.

While new to us, she’s not so new to English football!

Claire Willis

Claire joined the Penrose Health team in January this year, and we wanted to get to know a bit more about her. A keen F1 and football fan, she’s had the enviable experience of being at Wembley to witness England’s Lionesses crowned champions in the UEFA Women’s Euro 2022. Statistically, being there was almost a given, as she hasn’t missed an England game at Wembley for the past 20 years…

What's inspiring you in life right now? 

Seeing my niece chase her dreams is truly inspiring. She’s been acting for a few years now, and her hard work recently paid off when she won a Best Newcomer award. Watching her dedication and persistence reminds me that anything is possible with passion and perseverance.

Sweet or savoury? 

Savoury! I do enjoy sweet things, but I prefer natural sweetness over anything with added sugar.

What’s your favourite board game? 

My favourite board game is The Chase. I bought it for my family to play at Christmas, and it was such a hit! We had a great time, but let's just say we’re all a bit too competitive, so it got pretty intense!

What’s your guilty pleasure? 

Krispy Kreme glazed donuts—I love them and can easily eat three in a row! I know it totally contradicts my answer about preferring savoury over sweet, but some things are just too good to resist.

What’s one thing you had to learn the hard way? 

You can’t please everyone, and trying to will just exhaust you.

Where would we find you this weekend? 

If you had asked me last weekend, I’d have said the O2, watching Strictly Come Dancing: The Live Tour. This weekend, you’ll find me at the cinema and watching football. But if the weather plays nice, I’d much rather be out on a bike ride along the canal near my house. The scenery is beautiful, and the wildlife is amazing—Swans, Egyptian Ducks, Herons, and so many other birds. Plus, there’s always the odd coconut floating by, which is a bit of a mystery.

What’s your favourite underground line and why? 

The Central Line! Even though it’s been a bit unreliable over the last couple of years, it used to be one of the most dependable lines. What I love most about it is how well it connects with all the other Tube lines, making it easy to get anywhere in London.

If you were not a Contact Centre Service Supervisor, what would you be doing? 

I've always thought I’d be great at event planning—organisation and building relationships are my strengths. But luckily, these skills are just as valuable in my current role, so I still get to put them to good use!
